HealthBridge Insight Portal Terms of Service

1. Acceptance of Terms
By accessing or using the HealthBridge Insight Portal and its associated services (the "Service"), you agree to comply with and be bound by these Terms of Service ("Terms"). This Service is provided by HealthBridge, LLC ("HealthBridge," "we," "us," or "our"). If you do not agree to these Terms, you may not use the Service.
2. Description of Service
The HealthBridge Insight Portal is a clinical intelligence dashboard designed for authorized users (patients and care coordinators within the HealthBridge ACO network). The Service leverages the CMS Blue Button 2.0 API to retrieve, aggregate, and visualize Medicare Part A, B, and D claims data. This data is used solely for the purpose of improving care coordination, identifying gaps in care, and providing a comprehensive view of a beneficiary's health history.
3. User Responsibilities & Account Security
You acknowledge that you are responsible for maintaining the confidentiality of your login credentials (including your Medicare.gov authentication). Currently, the Service utilizes OAuth 2.0 authentication via Medicare.gov; we do not store your Medicare username or password.
You agree to:
- Provide accurate and complete information during the authentication process.
- Notify us immediately of any unauthorized use of your account or data.
- Use the Service only for lawful purposes related to personal health monitoring or authorized care coordination.
4. Data Usage and Privacy
Your use of the Service is also governed by our Privacy Policy.
- Data Retrieval: We retrieve your historical claims data (EOBs, Coverage, Patient info) only upon your explicit consent via the CMS OAuth flow.
- Data Storage: We do NOT store your Medicare data in any database or on our servers. Data is streamed securely to your browser via a Cloudflare Worker and is not retained after your session ends.
- Data Ownership: You retain ownership of your health data.
5. Medical Disclaimer
The Service is for informational purposes only. It does not constitute medical advice, diagnosis, or treatment. The insights provided are based on administrative claims data, which may has limitations (e.g., lag times, coding errors). Always seek the advice of your physician or qualified health provider with any questions you may have regarding a medical condition.
6. Blue Button API Notice
This product uses the Blue Button APIs but is not endorsed or certified by the Centers for Medicare & Medicaid Services (CMS) or the U.S. Department of Health and Human Services (HHS).
7. Termination
HealthBridge reserves the right to terminate or suspend your access to the Service immediately, without prior notice or liability, for any reason whatsoever, including without limitation if you breach the Terms. Upon termination, your right to use the Service will immediately cease.
8. Limitation of Liability
In no event shall HealthBridge, nor its directors, employees, partners, agents, suppliers, or affiliates, be liable for any indirect, incidental, special, consequential or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses, resulting from (i) your access to or use of or inability to access or use the Service; (ii) any conduct or content of any third party on the Service.
9. Governing Law
These Terms shall be governed and construed in accordance with the laws of the State of [State], United States, without regard to its conflict of law provisions.
